Insurance:

There are many different insurance plans as such we cannot guarantee coverage for a specific provider or service by each plan and are not responsible for charges incurred for providers or services not covered by a specific plan.

To confirm your benefits and coverage for services, please call the customer service number on your insurance card.

If we are not able to bill your insurance, self-pay options are available and we will provider you with a superbill to submit to your insurance for potential  reimbursement.

All of our providers can bill for Auto Insurance Injury claims / Personal Injury Protection

• Boulder Administrative Services, Inc. (Employer self funded plan)

  • Our doctors are in-network with Kaiser PPO plans - check with your specific insurance to  confirm benefits using our insurance checklist

  • Not accepting new patients for  HMO or Core plans or Medicare Advantage plans

• Lifewise

• Premera (not Premera Medicare Advantage Plans)

• Providence

• Regence (Including Medicare Advantage, but not MediGap- does not cover Naturopathic services)

• Uniform Medical

  • ( Regence Uniform Medical Secondary Medicare Plans  -

                        only plans with UDW Subscriber ID#)

For Out of Network/Non-Contracted Benefits we collect payment at your visit until we establish that insurance will pay when billed out of network.

• Health shares - we are able to bill some health share and health cooperative plans, check with your insurance for details

• Aetna - Out of Network Only

• Cigna - Out of Network Only 

• Meritain Health (independent subsidiary of Aetna)

• United Healthcare - Out of Network Only 

  • (No Apple Health or Community Plans)

• Medicare

  • Some Supplemental Medicare plans cover services offered at our clinic.
